Our Full Agreement
By enrolling in the MediSavers Program, you authorize National Health Plan
Corporation, or its assignee, to charge the credit card you have designated
in the application form for all medical, dental or other health provider
charges to participating providers. This applies whether the charges were
incurred by you, a member of your family, or any other authorized user.
You understand that all charges incurred by you or an authorized user will be
offset by savings when being charged to your credit card account. MediSavers'
fee will be one-quarter (in isolated instances, 33%) of your savings on every service. You may also incur
bank costs, currently 0% for debit cards, 1.65% of the transaction for
Discover and 2.0% of the transaction for Visa and MasterCard.
If your credit card does not have sufficient available credit to cover the
provider's fee, your claim will be rejected, and you will have to pay the
provider's usual and customary fee.
If your identification card for this program is lost or stolen, or if the
credit card you used to enroll is lost or stolen, you must notify the card
issuer and MediSavers.
There are no additional membership fees, monthly or annual charges
whatsoever. You may cancel your membership at any time by notifying
MediSavers at 1-800-647-2677. The MediSavers card is the property of the
issuer and must be returned upon request. The MediSavers card is only
available to active account holders of participating credit card issuers.
MediSavers reserves the right to alter or terminate this program at any time.
However, any credits due resulting from the use of the MediSavers card prior
to the termination of the program will be honored. The MediSavers program is
offered, operated and administered by National Health Plan and is not an
obligation of your credit card issuer. Any disputes concerning any discounts,
credits or awards under the MediSavers Program will not affect your payment
obligations to your credit card issuer on your credit card account.

MediSavers is an independent entity that contracts with providers on your
behalf. It is NOT a medical provider or an insurance company.
The participating providers, including doctors, hospitals laboratories,
dentists, pharmacies and opticians, are independent contractors, NOT
employees of MediSavers.
MediSavers does not participate in your medical care or instruct the provider
regarding your medical care. All care is rendered by the provider based on
their own training and experience, and is the sole responsibility of the
provider and not of MediSavers.
